Job Summary:

As a Care Coordinator you will be responsible for managing and supporting a specific assigned caseload of Nevada Partners clients, with developing individual plans and the resources required to bring their plans into action. 

 

Supervisory Responsibilities:

  • None.

Duties/Responsibilities:

  • Screen and assess the clients’ needs. 
  • Collaborate with fellow educators and experts. 
  • Assist clients with the development of their individual plans.
  • Complete all required documentation in a timely manner according to Nevada Partners requirements. 
  • Inputting data into required systems, ex. CRM, Excel… 
  • Establish and maintain communications with clients, staff, parents, and outside agencies. 
  • Manage tracking of cases through completion based on client’s plan. 
  • Provide required reports to management team. 
  • Participate in Nevada Partners sponsored events. 
  • Participate in Nevada Partners training and meetings. 
  • Perform additional duties as assigned.

  • Knowledgeable in conflict resolution and problem solving. 
  • Knowledgeable of community resources 
  • Ability to prioritize tasks. 
  • Ability to function well in a high-paced and at times stressful environment.
  • Proficient with Microsoft Office Suite

 

 

 

Education and Experience:

High school diploma or equivalent.

Bachelor’s degree in social work, counseling, or related field preferred. 

Two plus years in care coordination or case management. 

 

Work Environmental: 

Office setting in which team members have a strong sense of camaraderie and a good work ethic. 

 

Physical Requirements: 

Walking, standing, and prolonged periods of sitting at a desk and working on a computer.

Must be able to lift up to 15 pounds at times.
